DEP Sickness of Child FormSpecial paid leave due to illness of child aged 18 years and under (Dependant Employees)
DEP Fitness for Work StatementStatement of fitness for work. This Certificate must be handed to your supervisor/Line Manager within three days of the commencement of your sickness. A copy will be retained with your medical notes.
